New and expanded member programs effective January 1
Blue Cross and Blue Shield of North Carolina (Blue Cross NC) is committed to helping members manage their health through innovative and clinically proven programs. Effective January 1, 2025, the following programs launched to support eligible members with diabetes and weight management and tobacco cessation:
Tobacco Cessation powered by Pivot (Expanded Program): This program is open to eligible members and their covered dependents who are at least 18 years old and use any form of tobacco. Using behavior modification, Pivot Breathe offers quit support for all forms of tobacco-based products, including cigarettes, vape, and smokeless tobacco, regardless of what form is used. Eligible members will have access to a dedicated, expert trained Health Coach, and desktop access or a mobile app with behavior modification articles and games, supportive online community, and if eligible, an FDA-approved breath sensor* that provides real-time results of carbon monoxide (CO) levels demonstrating the positive impact of reducing or quitting smoking. Additionally, participants will receive up to 12 weeks of free Nicotine Replacement Therapy*
(*FDA approved only for quitting cigarettes).
Feed Your Health Diabetes powered by NourishedRx (Expanded Program): Feed Your Health Diabetes through NourishedRx is an innovative diabetes management program focused on eligible members 18 years or older who are managing Type 2 diabetes with an A1-C level of 8 or higher in the last six months. The program empowers participants to make meaningful lifestyle changes through high-quality, individualized, nutritious foods. The healthy food is sourced from local and national food partners to meet the needs of diverse populations. Eligible members will be paired with registered dietitians and wellness associates and receive prepared meals and meal kits, along with guided grocery shopping and meal prep help. Feed Your Health launched in 2024 for Individual Under 65 plans, Affordable Care Act (ACA) plans, and Blue Cross NC Employees, spouses, and dependents. Beginning January 1, eligibility expanded to include Commercial Fully Insured plans and Administrative Services Only (ASO) plans.
Providers can share this webpage with eligible members who can enroll and find additional information by visiting the Feed Your Health webpage. Members must be diagnosed with Type 2 diabetes and have a clinical A1C of 8 or higher in the last six months. Providers can provide a copy of labs through secure email to feedyourhealth@nourishedrx.com or fax to 917-540-2450.
Diabetes prevention and weight management powered by Vida (New Program): These programs offer personalized care to address the cardiometabolic spectrum through three distinct program options:
- Diabetes Prevention Program (DPP): This coach-led program focuses on behavioral change through self-monitoring and includes personalized fitness and food trackers, education and skill building, and 24/7 support. The program is available in English and Spanish. The DPP is available to Individual Under 65 plans, Commercial Fully Insured plans, and ASO plans.
- Weight Management: This program offers nutritional coaching and culturally adaptable eating plans, personalized support for meal prep and planning and behavioral change support through self-monitoring with devices and trackers, exercise, educational content and skill building and assessments, and 24/7 support. This program is also available in English and Spanish to ASO plans.
- GLP-1 for Weight Management: This registered dietician led program focuses on cognitive behavioral therapy (CBT) principles. It supports eligible members in gaining skills surrounding self-monitoring, medication adherence, nutrition support, physical activity, and emotional regulation. GLP-1 for Weight Management is available to members with a BMI of 30 and above without weight-related core morbidities or a BMI of 27 and above with weight-related core morbidities. For eligible members already on GLP1, Vida will honor the remainder of the prior authorization, and these members will still need to enroll with Vida. The GLP-1 for Weight Management program is available to ASO plans.
Members who are eligible to enroll in either the DPP, Weight Management or GLP-1 for Weight Management programs must also be enrolled in a Blue Cross NC pharmacy plan and will receive information directly from Blue Cross NC or Vida.
