Teladoc Diabetes Management Program Available to Medicare Advantage Members Effective May 1, 2024

Beginning May 1, 2024, Blue Cross and Blue Shield of North Carolina (Blue Cross NC) will offer Teladoc’s Diabetes Management program to Medicare Advantage members at no additional cost to the member.

Medicare Advantage members who enroll in the program will receive a blood glucose monitor with unlimited strips and lancets shipped directly to the member’s home. Members can easily order refill test strips and lancets using the meter. The meter uploads readings to a secure member account for easier tracking and reporting that can be shared with a provider. Additionally, personalized tips are sent out after each check to help improve blood sugar. Teladoc’s coaching team observes blood glucose levels over time and provides suggestions for reducing A1C and disease burden, as well as addresses any urgent concerns. These members will also have access to one-on-one coaching, if desired.

Medicare Advantage members who are at least 18 years of age, have claims data identifying them as having Type 1 or Type 2 diabetes in the last 24 months, and have Blue Cross NC as the primary payer from a Coordination of Benefits (COB) perspective will be eligible for Teladoc’s Diabetes Management program.

Eligible members will receive marketing materials from Blue Cross NC and Teladoc encouraging them to enroll after the program has launched.

Providers and care managers internal to Blue Cross NC can help eligible members to start using this program by directing them to call Teladoc toll-free at 1-800-835-2362 (TTY: 1-855-636-1578) 24 hours a day, 7 days a week using registration code: BCNCDM or to visit www.teladochealth.com/explore/BCNCDM.
