Skilled Nursing Facility (SNF) Quality, Efficiency, and Outcomes
Blue Cross and Blue Shield of North Carolina (Blue Cross NC) strives to ensure that members have access to providers and facilities that deliver high quality, high value care throughout their health care journey.
To support this, we continue to evaluate the performance of our Skilled Nursing Facilities (SNF) through outcomes data such as readmission rates, emergency department utilization, average cost per episode, and efficiency metrics when evaluating participation. Additionally, we’re striving for a SNF network of facilities who achieves and maintains a Centers for Medicare & Medicaid Services (CMS) Overall SNF Star Quality Rating of 3-5 stars.
