At-home Medical Care to Transition from Landmark Health to ConcertoCare

Effective October 1, 2022, Blue Cross and Blue Shield of North Carolina (Blue Cross NC) will begin transitioning at-home medical care of Medicare Advantage seniors and other adults with unmet well-being needs from Landmark Health to ConcertoCare. Blue Cross NC will officially end working with Landmark Health by the end of 2022.

ConcertoCare is a tech-enabled, value-based provider of at-home, comprehensive care for seniors and other adults with unmet medical, behavioral and social needs. ConcertoCare orchestrates interdisciplinary teams of nurse practitioners, registered nurse care managers, social workers, health coaches, and pharmacists to deliver at-home and virtual comprehensive care.

Using a combination of in-person and virtual member engagement, ConcertoCare’s multi-disciplinary care teams are able to deploy a comprehensive set of services that are tailored to the unique needs of our members.

Currently enrolled members utilizing Landmark Health can continue to call Landmark Health and receive services until they transfer to ConcertoCare. ConcertoCare will contact eligible members when it is time for them to transition from Landmark Health.

Providers should continue to work with Landmark Health to ensure their plan of care is being implemented. ConcertoCare will contact providers if any members are eligible for their in-home care program.

Please note, if eligible, some members may also transition from Landmark Health to a Blue Cross NC Care Manager. Blue Cross NC representatives and/or other clinical partners will contact these members directly.
