Their goal is to provide the personalized care you deserve. They can answer any health questions you might have, find useful resources, and help you navigate the health care system. The Nurse Support Program is included with your plan, so there is no additional cost for you.
If you are living with or have been recently diagnosed with a chronic condition (such as diabetes or heart disease), it might feel challenging to manage. Your doctor can provide a lot of helpful information, but sometimes it can be hard to remember what to do once you leave the doctor’s office, or between visits. This is where the Nurse Support Program can help support you.
It’s really easy to reach your Nurse Support Program team. You can call them on the phone or send them secure messages using a mobile app called Wellframe®. Whether you have a question or an update on your health, with Wellframe, you can send a secure message on your own time, whether you are at home or on-the-go. You can opt out or decline to participate at any time.
