Blue Cross and Blue Shield of North Carolina (Blue Cross NC) is continuing to communicate the importance of having an Annual Preventive Visit to Medicare Advantage members. Starting mid-May and continuing in late-July, Medicare Advantage members will receive personalized communications with a Health Care Status checklist showing them which visits and screenings they should be getting, which of those visits and screenings they’ve completed and which they still need to complete. It also indicates which of the screenings are rewardable.
Where applicable, members’ personalized Health Care Status checklists will also include information about medication adherence. The checklist is based on age, gender and Diabetes diagnosis. Care gaps are based on claims, and Diabetes care gaps follow Healthcare Effectiveness Data and Information Set (HEDIS) guidelines.
We are encouraging Medicare Advantage members to schedule the following visits and screenings that are applicable to them.
- Annual Wellness Visit, Welcome to Medicare Visit, or Annual Routine Physical
- Colorectal cancer screening
- Breast cancer screening
- Diabetes screenings:
- Retinal eye exam
- Kidney/renal function exam
- A1C blood test
- Bonus for completing all three diabetes-related exams/tests
We are suggesting members take the Health Care Status checklist with them when they see their primary care provider.
We encourage providers to discuss this checklist, physical health, mental health, and the risk of falls and bladder control with our Medicare Advantage members when you next see them.
