Your primary care provider (PCP) knows your health care needs best. That’s why scheduling your Annual Wellness Visit,1 Welcome to Medicare Visit,2 or Annual Routine Physical with your PCP is so important. Whether your PCP is a doctor, nurse practitioner, or physician assistant, they work with you to:
- Assess and monitor your health
- Conduct screenings and administer essential vaccines
- Discuss your health risk factors and strategies to best manage them
- Schedule this visit with your PCP.
- Specifically mention that you are scheduling an Annual Wellness Visit, Welcome to Medicare Visit, or Annual Routine Physical in order to qualify for your $50 preventive care reward.
- To ensure you and your provider can focus on preventive care, make sure you aren't sick or in pain on the day of your preventive visit. If you are, communicate this with your provider so they can treat what ails you. Be sure to reschedule your annual preventive visit for another day.

Your PCP knows your health history, current health conditions, risk factors, and your prescription medications. Whenever you seek care, it’s important to start with the provider who knows you best to avoid unnecessary provider visits or potential negative medication interactions. Based on what they know about you, they can also refer you for additional care with the right provider for your unique needs.

Ask your provider about virtual care!
There’s no place like home when you’re not feeling well. Check to see if your provider offers virtual visits by phone or video. If so, virtual visits are covered the same as in-person visits according to your plan.
Note: Preventive visits (Annual Wellness Visits, Welcome to Medicare Visits, and Annual Routine Physicals) must be in person.
