Your Annual Preventive Visit is either:

  • an Annual Wellness Visit
  • a Welcome to Medicare Visit
  • or an Annual Routine Physical

As a part of your Blue Cross and Blue Shield of North Carolina (Blue Cross NC) plan, we encourage you to have an Annual Wellness Visit and an Annual Routine Physical each year. You will receive one $50 gift card for the first annual preventive visit you complete.

About Annual Preventive Visits

Your Annual Wellness Visit or Welcome to Medicare Visit (for people who have enrolled in Medicare in the last 12 months)1 focuses on preventing or managing disease and disability. It’s a discussion with your primary care provider (PCP) – the doctor, nurse practitioner, or physician assistant that you see regularly for most of your medical Preventive Visit needs – where you work together to ensure you’re on the path to your best health.

Your Annual Routine Physical involves:

  • An exam to check how well your body is functioning
  • Screenings and vaccines you may be due for
  • Conversations about your risk factors for health problems 

Schedule this visit when you’re not sick, hurt, or having a flare-up of a chronic condition.

Remember, your Annual Wellness Visit, Welcome to Medicare Visit, or Annual Routine Physical must be with your PCP. Also, when scheduling your visit, use the exact words: “Annual Wellness Visit,” “Welcome to Medicare Visit” or “Annual Routine Physical.” Otherwise, the scheduler may think you are making an appointment for a specific condition or concern. After your visit, we will send you instructions on how to claim your $50 gift card. (Please allow up to 12 weeks to receive your gift card.)

Be sure to take advantage of your Annual Wellness Visit or Annual Routine Physical once a year. We will send you a $50 gift card for the first annual preventive visit you complete. These are great ways to protect and maintain your health.

NOTE: You can receive one $50 gift card per year for having any one of these three visits. You cannot earn more than $50 by having multiple visits from this list.

We may be able to help you get to your Annual Preventive Visit

Your plan may include services such as no-cost transportation to and from your appointments. Check your evidence of coverage to see what's available to you.

Members with diabetes can earn $30 more for completing 3 more screenings

People with diabetes need regular screenings to manage their conditions. Members with diabetes can earn $30 more if they complete all 3 of these screenings:

  • Retinal eye exam – a full eye exam with dilated pupils (you can find a vision provider via Blue Connect)
  • A1C – a blood test to measure blood glucose (sugar)
  • Kidney health evaluation – a set of blood and urine tests to measure kidney function

This $30 reward can be combined with your Annual Preventive Visit reward for a total of $80. During your Annual Preventive Visit, talk with your doctor about these and other screenings you may need.

Enroll in Blue Connect to be eligible to receive your reward

If you already have a Blue Connect℠ account, no further action is needed. We will send you a postcard with instructions to claim your reward after your Annual Preventive Visit and / or diabetes screenings.

Your PCP: The doctor at the heart of your health care team

It’s important to have a PCP that you see regularly for most of your medical needs. PCPs handle preventive care, routine tests and most other common treatment. Your PCP is also who you can go to with any health questions or concerns. They work with any other health care providers or specialists you see to make sure your health is taken care of from every angle. 

To make the most of your health and your Blue Medicare℠ plan, if you don't have a PCP yet, log in to Blue Connect and use the Find a Doctor tool to search for an in-network PCP near you, or call the Customer Service number on the back of your member ID card.

Ask about virtual care

Many health care visits can happen virtually! Whether by phone or online video, virtual visits are covered the same as in-person visits according to your plan. If you’re feeling sick, virtual visits are a great way to get health care from the comfort of home while protecting yourself and others.

Ask your PCP about virtual appointment options when scheduling your Annual Wellness Visit. (Welcome to Medicare Visits and Annual Routine Physicals must be in person.) Also, if there’s any care you’ve been delaying, consider making a virtual appointment rather than waiting any longer. 

(If you’re feeling unwell, call ahead to your doctor's office before showing up in person.)

Disclosures:

Your eligibility for the gift card is based on claims data, so be sure your appointment is scheduled as an Annual Wellness Visit, Welcome to Medicare Visit, or Annual Routine Physical. Your doctor must also file the claim as this kind of visit.

Please allow at least eight weeks from when we receive your claim (up to 12 weeks including processing) before calling to inquire about your gift card.
