Managing your health care alone can be hard, especially if you have multiple health problems.
You'll have a case manager who will work with you and your doctors to make sure you get the right care. These case managers are nurses, social workers, and mental health clinicians.

You can get no-cost rides to and from your appointments. Healthy Blue + Medicare plan includes unlimited rides to grocery stores,8 pharmacies, and doctors' offices with a $0 copay.3 Rides are limited to 60 miles each way.
