Case management

Managing your health care alone can be hard, especially if you have multiple health problems.

You'll have a case manager who will work with you and your doctors to make sure you get the right care. These case managers are nurses, social workers, and mental health clinicians.

Transportation services

SafeRide℠ Health makes it easy to get to and from doctor appointments, your pharmacy, and the grocery store.1 SafeRide matches you with the transportation type that meets your needs, including wheelchair accessible vans and non-emergency ambulances. You can book in advance or up to two hours before your appointment. This benefit includes 48 non-emergency, one-way rides per year.5

You may qualify for Special Supplemental Benefits for the Chronically Ill (SSBCI) if you are at high risk for hospitalization or adverse health outcomes and require intensive care coordination to manage chronic conditions such as cardiovascular disorders, cancer, stroke, diabetes or chronic lung disorders. Eligibility must be established before the benefit is provided and cannot be guaranteed based solely on your condition. For a full list of covered chronic conditions or to learn more about eligibility requirements, please contact your plan.

  2. The $0 premium is for Healthy Blue + Medicare members who live in the approved service areas.

  3. All Medicare Advantage members are required by the federal government to continue paying their Part B premium.

  4. $0 copays for in-network primary care providers. For more information, contact the plan.

  5. Must use designated providers.

  6. Service limitations apply. Members also have a $0 cost share when services are provided by non-participating dentists; however, out-of-network/non-contracted providers are under no obligation to treat plan members, except in emergency situations. Please see the Evidence of Coverage for more information.

  7. $0 copay for drugs in Tiers 1, 2 and 6. For Tiers 3, 4 and 5, copays depend on Low-Income Subsidy (LIS) eligibility. Applies at in-network pharmacies.

  8. Does not roll over from month-to-month.
