Blue Cross and Blue Shield of North Carolina (Blue Cross NC) has updated the online form link for behavioral health care navigation referrals, as follows:

To ensure smooth processing, please delete any previously saved or bookmarked referral links and replace them with the new form link above. You can also continue to access the care navigation referral form via Blue e or the Blue Cross NC website. Providers and members can also request a care navigation referral by calling 800-755-0798.
The behavioral health care navigation referral service is available to Commercial plans (including Administrative Services Only), the Federal Employee Program, Medicare Advantage plans (including Experience Health), and the Dual Eligible Special Needs Plan (D-SNP). The service is not available to Healthy Blue members or Medicare Supplement plans.
