Effective May 1, 2026, providers must submit all Commercial and Medicare provider coding disputes using the Commercial and Medicare Provider Coding Disputes form (PDF). This form is required to ensure your dispute is routed correctly and processed without delay.
In addition, providers must continue using the Commercial Level One Provider Appeal (PDF) and Medicare Provider Appeal (PDF) forms exclusively for provider medical necessity reviews. This form should not be used for coding disputes, as doing so may result in returned submissions.
We encourage providers to submit these requests through the Provider Portal (Blue e), as it allows for faster submission and tracking, and no paper form is required when you submit electronically.
If you choose to submit a paper coding dispute, send it to the following newly established PO Box, which is dedicated specifically for Commercial and Medicare provider coding disputes, effective immediately:
