Genomic Testing for Hematopoietic Neoplasms AHS- M218212/5/23 New policy developed. Clinical information and criteria from M2027 BCR-ABL1 Testing and M2062 Genetic Testing for Acute Myeloid Leukemia moved to this new policy. Includes most recent NCCN guidelines. Medical Director review 10/2023. Notification given 12/5/2023 for effective date 2/6/2024.
