Termination request for dependent coverage
  • If you wish to terminate dependent coverage, please submit the request online by choosing “Cancel Dependents” in your account on the Student Blue portal.
  • Termination requests must be submitted 10 days prior to the first day of the month that coverage is no longer desired.
  • Terminations are available on a monthly basis and effective on the last day of the month.
  • There are no refunds.

The following is a summary of some of the insurance coverage restrictions for student health plans. A full explanation and listing of restrictions can be found in your Benefit Booklet. 

Your health benefit plan does not cover services, supplies, drugs, or charges that are:

  • Not medically necessary
  • For injury or illness resulting from an act of war
  • For personal hygiene and convenience items
  • For inpatient admissions that are primarily for diagnostic studies
  • For palliative or cosmetic foot care
  • For investigative or experimental purposes
  • For cosmetic services or cosmetic surgery except as specifically covered by your health benefit plan
  • For custodial care, domicillary care or rest cures
  • For treatment of obesity, except for surgical treatment of morbid obesity, or as specifically covered by your health benefit plan
  • For reversal of sterilization
  • For treatment of sexual dysfunction not related to organic disease
  • For conception by artificial means
  • For self-injectable drugs in the provider’s office

Deductibles, coinsurance, limitations, and exclusions apply to your coverage. Further details of coverage, limitations, and exclusions, and terms under which your policy can be continued in force will be provided in your benefit booklet.

Waiver Request – Domestic Students only

 

Students who are covered on a health insurance plan of their own, or a parent’s plan, may elect to waive the Student Blue plan if the current plan offers credible coverage and meets the following criteria (within given deadlines):

1. The plan must cover, at a minimum, access to emergency health care in the Elon, Burlington, Alamance County areas. You will be responsible for any charges not covered by your insurance.

2. The plan must remain in effect while enrolled at Elon University.

 

Do you have another plan that meets the criteria above?

Waive the university plan by submitting evidence of coverage. You must complete all steps of the waiver and get a confirmation email.

 

You will need:

1. Your Unique ID number (located in OnTrack under the User Options button on the left, then choose User Profile, located under your picture).

2. Your current insurance information, including insurer name, phone, and policy number.  

Note: If the policy holder is someone other than the student, you will need the policy holder's full date of birth.

3. A picture of the front and back of the insurance card.
