Your 2023 Medicare Advantage plan includes several programs and resources to help you stay as healthy and independent as possible. Whether you need a ride to the doctor, in-home care or even a healthy meal, we’re here for you. See what’s offered below – and be sure to share it with loved ones and caregivers, so they also know what’s available.

Caregiver tools and support

Carallel provides support to family members caring for their loved ones.

The MyCareDesk® digital platform offers tools and resources to help make decisions about senior living, in-home care, finances and more. You can also use the platform to store documents and share calendars and medication information with others in your circle of care.

For live support, members or their caregivers can call, email or chat with one of Carallel's highly trained Care Advocates, who can help them navigate difficult or confusing situations.

Personal Emergency Response System (PERS)

Your plan includes a no-cost wearable PERS device. With the press of a button, you’re connected to the call center at Connect America. They then connect you with the emergency services you need.

Most devices also include:

  • Fall detection
  • GPS tracking
  • An app to alert family or caregivers

Meals delivered to your door

When recovering from a hospital stay, you can get the nutritious meals you need once you get home. Mom’s Meals provides 14 days of prepared meals. All you need to do is heat and eat.

How it works

Upon discharge from a qualified stay:

  • A case manager will contact you to ask if you want to access the benefit
  • The case manager will send a referral to our meal vendor, who will call you to set up delivery
In-home assistance

The CareLinx network of pre-screened, professional caregivers provides you with extra help in your home when you need it.1

CareLinx's staff works with you to understand your needs, schedule and caregiver preferences. Within one to two weeks, you’ll be matched with candidates who best meet your needs.

The program includes:

  • Personalized caregiver matching
  • 60 hours per year of in-home support services
  • Help with meal prep, bathing, medication reminders and more

Non-emergency medical transportation

Never miss another medical appointment. SafeRide℠ provides flexible options to book a ride in advance or on-demand. They will match you with the transportation type that most closely fits your specific needs.1

SafeRide offers:

  • 24 one-way rides per year
  • Wheelchair-accessible vans and non-emergency ambulances also available
  • Real-time status updates via SMS (text)
