Blue Cross NC will be distributing resources within our network of Accountable Care Organizations (ACO) that have high-volume practices with identified quality improvement opportunities.
To support our shared 2025 Quality Improvement efforts, beginning in July of 2025, we will provide standard one-pagers to targeted practices, focusing on key Medicare Advantage Stars measures including:
- Annual Preventive Visit (APV)*
- Controlling Blood Pressure (CBP)
- Eye Exam for Patients with Diabetes (EED)*
- Glycemic Status Assessment for Patients with Diabetes (GSD)*
- Statin Therapy for Patients with Cardiovascular Disease (SPC)
- Statin Therapy for Patients with Diabetes (SUPD)
*Indicates member incentives available upon gap closure/member attestation
These one-page resource sheets will dig deeper into claims submission, patient education, and best practices for workflow optimization, which can be leveraged practice-wide.
