Experience Health Member Case Management Services Transition from Duke Health to Blue Cross NC Case Management Team November 04, 2024 Products & Programs Physicians & Specialists Facilities & Hospitals

Blue Cross and Blue Shield of North Carolina (Blue Cross NC) has made the decision to transfer case management and pharmacy quality services from Duke Health to our internal Case Management and Pharmacy Quality teams to meet the needs of eligible Experience Health members.

Proper coordination of care with primary care providers can help members manage conditions, reduce adverse events, and avoid trips to the emergency room. Blue Cross NC has a dedicated case management team that follows members with complex needs and assists with care planning to improve physical and emotional health.

The transition will take place through the remainder of 2024, and Blue Cross NC will assume all case management services by January 1, 2025. There are no actions required from our members for this transition of services, and they do not need to re-enroll or reapply for services.

During the transition phase, members can continue to use the Duke Health contact information, and Blue Cross NC Customer Service number for member support and continuity of services. Current care plans will continue without interruption during the transition plan.

Blue Cross NC’s Case Management team will carefully review all members’ information before making outreach calls about the transition. Member information will also be carefully reviewed to determine if they are at a high risk and/or qualify for our multiple case management programs and resources.
