Guidelines for submitting tax ID and legal name change requests to Blue Cross and Blue Shield of North Carolina (Blue Cross NC).

How to submit change requests

These instructions are applicable to all submittals.

  • Email requests to providerupdates@bcbsnc.com. You can also fax your requests to 919-765-4349. Processing time can vary between requests sent to Commercial or Blue Medicare. Average processing time is between 10 to 30 business days.
  • Include complete provider details such as name, NPI, IRS number, locations and Medicare number. You must provide the effective date.

Incomplete requests will not be accepted for processing. Requests that are incomplete or are missing information will be returned to sender. You will need to resubmit the request with complete information to be processed.

Tax ID change with no legal name change
You must submit requests 30 days prior to the termination date and include:
  • Completed demographic form or request on letterhead
  • Updated W9
  • A new EFT form for participating groups
Legal name change
You must submit requests 60 days prior to the effective date and include:
  • Completed Provider Demographic Change Form (PDF) or request on letterhead
  • Updated W9
  • For contracted participating groups
    • A new contract or contract amendment will be required
    • The legal name change must be on file with the IRS for validation
Tax ID and legal name changing due to a merger or acquisition
You must submit requests 60 days prior to the effective date and include:
  • Completed group enrollment application
  • Updated W9
  • Termination letter or bill of sale from previous owner
  • Individual enrollment applications for all practitioners who are not currently active in Blue Cross NC
  • Current medical license for practitioners who are not currently active in Blue Cross NC
  • A new EFT form for participating groups
  • For contracted participating groups
    • Current contract must support specialty of new practice
    • All practitioners must be credentialed (if applicable) in order to be enrolled
    • It may be required that a new contract or contract amendment must be signed prior to making the above changes. This may impact the effective date requested.
