Review the steps, required documentation, and timelines to remove a provider from your group or terminate your group’s contract with Blue Cross and Blue Shield of North Carolina (Blue Cross NC).
You must report a practitioner leaving a group at least 30 days before the termination date or as outlined in your agreement or attached practitioner roster.
Submit a formal request on your official practice letterhead that includes:
- Reason for removal (e.g., retired, left practice, moved out of state)
- Continuity of care plan with any neccessary moves to ensure member care
- Signature:
- Practitioner’s signature if they request removal
- Authorized group representative if the group requests removal

You must report contract terminations at least 90 days before the termination date or as outlined in your agreement.
Submit a formal request on your official letterhead that includes:
- Reason for termination (e.g., retired, closed practice, network status change)
- Signature:
- Practitioner's signature if they request termination
- Authorized group representative if the group requests termination
