Our Healthy Blue + Medicare (HMO-POS D-SNP) case managers make it easy for you to get the care and help you need.

 Supporting your health care

As a Healthy Blue + Medicare℠ member, you have a dedicated case manager to support your care. The case managers are nurses, social workers, and mental health clinicians who can help you get the services you need.

This no cost, optional service is included in your plan and gives you access to helpful resources like:
A care plan made for you

You will be contacted to complete a Health Risk Assessment that helps your case manager understand your health needs and how they can best support you. They will then create a customized care plan for you.
Care coordination

Keeping everything straight can be difficult when you have multiple doctors. Your case manager will coordinate your care with all of your doctors. They can also help you find doctors, pharmacies, and services near you.
Help using your benefits

In addition to medical, prescription drug, vision, and dental care, your Healthy Blue + Medicare plan comes with other benefits like food support and transportation services. Your case manager will help you make the most of your plan.1
Answers to medication questions

If you have a question about your medication after leaving your doctor's office, your case manager can help you get clear and simple answers quickly.

Knowing how and where to get medical supplies and equipment can be confusing. We can help you get the supplies needed for your health conditions.
