As a Healthy Blue + Medicare℠ member, you have a dedicated case manager to support your care. The case managers are nurses, social workers, and mental health clinicians who can help you get the services you need.

You will be contacted to complete a Health Risk Assessment that helps your case manager understand your health needs and how they can best support you. They will then create a customized care plan for you.
Keeping everything straight can be difficult when you have multiple doctors. Your case manager will coordinate your care with all of your doctors. They can also help you find doctors, pharmacies, and services near you.
In addition to medical, prescription drug, vision, and dental care, your Healthy Blue + Medicare plan comes with other benefits like food support and transportation services. Your case manager will help you make the most of your plan.1
